Initial Intake Assessment

Service Description

The initial intake session includes: -Review of consent forms -Review of confidentiality, HIPAA, and privacy practices -Overview of clinician's background and areas of expertise -Discuss presenting concerns and reasons for therapy with the clinician -History of presenting concerns -Family History -Mental Status Exam -Review of Strengths and Limitations -Receive recommendations for treatment -Request prior authorization to conduct therapy if needed

Cancellation Policy

You may find that you need to cancel an appointment due to unforeseen circumstances. Please notify us as soon as possible so that we may offer the time to someone who may be waiting for an appointment. We ask that you cancel or reschedule your appointment at least 24 hours in advance of your scheduled start time to avoid being charged one half the fee for that appointment.
